Playbooks on session ensure; Memory explorer shows recall

What shipped.

• memory_session_ensure can take work_id / playbook_slug and injects the named playbook into the ensure payload (same compose path as agent_run_start). Grok Bot and other harnesses that bootstrap via ensure now get the work playbook.
• Memory explorer lists Last recalled (relative or Never) and Recalls. Detail pages show the same.
• Never-recalled filter on the explorer so you can find unused durable memories before eviction.
