SessionStart soul/playbook inject; Codex logos; install README fix

September 15, 2026

What shipped.

• Cursor sessionStart now injects TeamShared soul and a short playbook header into additional_context after memory_session_ensure (empty soul + no playbook stays quiet; fail-open on MCP errors).
• ChatGPT/Codex package 0.13.1 ships triangle logo assets so Sources shows the mark.
• Plugin README no longer points at the private teamshared-ai/teamshared 404; install docs use teamshared.com/#connect, this plugin repo, and /mcp.
