GitHub connect, Agent errors, Knowledge trust, and plugin recall

- Connect your GitHub account once from Settings. Agent checkouts prefer that connection for private repos, and Disconnect asks for confirm first.
- Failed Agent runs show a short error under the row. An empty checkout folder is no longer treated as a successful clone.
- Knowledge lists memory as documents with kind, who wrote it, and clearer article labels so curator pieces are not mistaken for unlabeled facts.
- Memory recall is sharper: query embedding matches how passages were stored, relative dates resolve, skill search ranks before the limit, and connector re-imports stop writing duplicate copies of the same source.
- Recall hits carry a small provenance block agents can cite. On the Cursor / Claude / Codex plugin, SessionStart can pull a compact Recalled list, and a failed Cursor tool can surface prior org fixes before retry.
- Hermes on Railway should use a seat bearer token (
tsk_…), not OAuth — TeamShared does not allowlist*.up.railway.appcallbacks.
